The State of Minnesota has "direct access" which means that you can come to Fitness Focus for an injury without seeing a medical doctor first. If you need care beyond 90 days, a referral would then be required to obtain a confirming diagnosis to continue treatment. 

*An exception would be those whose primary insurance is Medicare. Medicare requires a referral for treatment prior to seeking Physical Therapy. 


YES.  The choice of where you want to receive physical therapy is always yours. Some patients come to us after being referred from their physician and others request to see us as a result of our strong reputation in the community.


Fitness Focus is an authorized provider for several insurance plans. Currently we accept Aetna, Blue Cross Blue Shield, Health Partners, Cigna, Medicare Part B, Railroad Medicare, Tricare, Medica, Preferred One, UCare for Seniors, Humana, Worker's Compensation, and All Auto Insurance. 

Due to the many changes in insurance policies, it is no longer an easy task to interpret each individual policy. Therefore, we ask you, the patient, to please check with your insurance company regarding your coverage. It is your responsibility to know your individual coverage and limitations. Some plans require a referral from your primary care physician. Failure to comply with this suggestion could result in you, the patient, being responsible for all costs incurred. Please remember that your insurance policy is a contract between you and your insurance company. 

We also offer a competitive self-pay rate as an alternative to out of network rates. This option can be an affordable solution to ever rising deductibles and copays. Self-pay reduces administrative costs and eliminates the long delays in claims processing which saves time and money for both parties. With self-pay, you can submit the claim to your insurance company for possible reimbursement. HSA and Flex accounts can also be used for Physical Therapy services.
